What participation entails
To participate in Stap Vooruit – a customized participation program, you will complete two questionnaires: one at the start of the project and one at its completion. The questions concern your participation in society and the impact of your symptoms on it. You will receive the results via your electronic patient file in Solvio.
In addition, Q-support and C-support use your responses to both questionnaires for evaluations: we want to analyze the impact of the program to gain more insight into the interventions that are particularly successful. Based on the results and insights gathered, we make recommendations to the Ministry of Social Affairs and Employment and other stakeholders. Your responses will be combined with those of others and processed anonymously, so no one can tell the data originated from you.
When processing and providing your data, we comply with the General Data Protection Regulation (GDPR). You can find out exactly what this means for you in the privacy statement Read. Your collected data will remain available for ten years after the end of this project, unless you withdraw your consent at any time.
If you participate in the participation process, we ask the following of you:
- If you are not yet registered with Q-support or C-support, we ask your permission to register with our organization.
- If you do not yet have a file with us, we will ask for your permission to create an online patient file. Solvio to create. Solvio is a secure online environment of Q-support and C-support.
- Consent for the processing of your personal data by Q-support and C-support in the manner described in the privacy statement.
- Permission to link your questionnaire answers to your online file in Solvio so that the answers to your questionnaires become visible in this system.
- Permission to link conversation reports with your case manager and involved (healthcare) professionals to your online file in Solvio. These conversation reports are not part of the analysis.
- Consent to use your questionnaire responses for analysis and evaluation of the project, where your responses will be combined with those of others and processed anonymously so that no one can see that the data originated from you.
- Consent to use your questionnaire responses for research purposes and to link them to data collected within Q-support and C-support. Your responses will be processed anonymously so that no one can see that the data originated from you.
Possible advantages and disadvantages
If you participate, after completing the questionnaire, you will receive an intensive, personalized 6-month participation program. During this program, you will have 10 to 12 (telephone) sessions of 45-60 minutes each to guide you toward greater participation in society. This support takes time and energy and can be challenging.
If you no longer wish to participate
In this program, we aim to investigate the effects of the intensive program, so that we can gain more insight into the interventions that lead to particularly good results. You decide whether to complete the questionnaire and participate in the program. Participation is voluntary, and you can stop at any time during the program. You can do so by sending an email to stepforward@c-support.nu stating that you are withdrawing your participation. You do not need to provide a reason.

- I agree to link my data to other questionnaires within Q-support and C-support (if I participate in them) for further analysis and to gather insights.
